CONFIDENTIALITY AGREEMENT

BY SIGNING THIS FORM, I AGREE TO THE FOLLOWING CONFIDENTIALITY GUIDELINES:

I will not discuss or reveal any specific information about the case(s) I hear or work on when I am participating in the Peer Court program.

I will not reveal the respondent’s, victim’s or any witness or family member’s full name or any identifying information about the above individuals or their involvement in the case outside the hearing or hearing preparation unless I am discussing the case with Peer Court staff or other youth or adult participants.

I understand that if I speak with Peer Court staff or other youth or adult participants about the case(s), it must be done in a private setting where no one else can hear the discussion and I will use first names only.

I understand that if I fail to uphold the confidentiality agreement, I will be held accountable to the Peer Court program and may receive consequences for my actions.

I understand that Peer Court expects me to be accountable for my actions in the same way it expects respondents to be accountable for theirs.
