AGREEMENT TO PARTICIPATE IN PEER COURT

 

Greetings-

To allow your minor child or ward to participate in Peer Court, please sign this document and return it to Peer Court Staff or Juvenile Probation Department Staff.

 

We are contacting you because you have been listed as part of an incident that occurred in San Francisco and was referred to the San Francisco Peer Court program by the SF Police Department, Juvenile Probation Department or School District. 

 

San Francisco’s Peer Court program works to help victims, offenders, families, community members and law enforcement to come together to resolve conflicts and incidents that have happened at schools or in the community. Peer Court is a community-based and Peer Led program funded by the City and County of San Francisco.

 

Your participation is voluntary, but in order for you to participate, you must agree to the following:

1.    Sign an “Agreement to Participate in Peer Court” Form

2.    Allow Peer Court to contact you regarding this incident and to invite your participation

3.    Allow Peer Court access to documents that contain private information, such as:

a.    Your private contact information such as name, telephone number, address, etc.

b.    Details about the incident, such as police reports or summaries, photographs, etc.

By signing this document, I hereby understand and agree to the following terms:

 

  1. I understand that Peer Court is voluntary. I can choose to participate or not to participate.

 

  1. I agree to grant Peer Court access to my contact information and documents pertaining directly to the incident that has been referred to Peer Court. I understand that this information is otherwise confidential, unless I agree to share it.

 

  1. I agree to treat all abide by all Peer Court ground rules, and will treat all participants with respect at all times. Ground rules include:
    1. Treat everyone with courtesy and respect.
    2. No fighting, yelling, name-calling or other disrespectful behaviors.
    3. Tell the truth.
    4. Work together to reach an agreement.
    5. Do your best to complete your part of the agreement by the date specified.
